How do I apply for services?

   1. Tribal enrollment (Certificate of Indian Blood, Tribal I.D. Card or other document
verifying enrollment in a Federally recognized tribe).
   2. Proof of Residency in Maricopa County, off-reservation, such as current driver’s license or AZ ID card, rent/lease document with current address, mail addressed to applicant, etc.
   3. Payroll check stubs for the last six (6) months for applicant and spouse and/or adult children in the house if they have been employed within the previous 6 months.
   4. Selective Service registration for men between the ages of 18 – 26 (NWS can obtain Selective Service registration through the Internet if you have previously registered.
   5. Social Security Card
   6. AZ Driver's License or I.D.
